Founded in 2019, ReLeaf Medical is currently located at 5615-B Jackson Street Ext., Suite 102 in Alexandria, LA 71303. We aim to serve our clients, providing the best treatment and solutions through the increasingly prevalent method of medical marijuana.

Payment

Patients usually have in-person appointments every few months, with initial consultations varying. Consultations all cost $200 and take a minimum of forty minutes to complete. The doctor will evaluate and confirm your eligible medical condition in addition to reviewing possible treatment plans and ideal outcomes. Through this evaluation, our doctors are able to create personalized plans for treatment, in addition to answering any and all questions about possible treatments and methods involving medical marijuana.

The state of Louisiana currently restricts recommendations for medical marijuana to a limit of thirty days, meaning that it is typical that patients come in for refill appointments to continue and ensure proper treatment. These appointments for refill can be done over the phone, at the patient’s request. Phone appointments last between 5-10 minutes and can cost up to $50.

Other Services

 Other charges may arise that are separate from the non-refundable initial deposit or the cost of the consultation. These charges usually involve paperwork, organization with your physician’s office, or appointments running over the allotted time.

 It is important to specify that ReLeaf Medical acts as a clinic to provide recommendations for medical marijuana treatment for patients in Louisiana, opting into Louisiana’s medical marijuana program. No other medications or prescriptions are given out at our clinic.

Scheduled Appointments

All patients must have an appointment, scheduled a minimum of a week prior to the date of the appointment. A non-refundable initial deposit is required when scheduling an appointment because of the personalized care and specificity that we give to each new patient. Cancellations, not showing up, or changing the time of a an appointment will incur the price of the deposit and a supplemental fee.

 

What to Bring

 Please bring a valid government-issued photo ID, such as a passport or driver’s license. All the information on the ID must be valid and there must be a picture on the ID that authentically displays a picture of yourself.

 In addition, we require evidence from the past year of your residence in Louisiana. Recent medical records (also within the last year) are also required from your present physician to certify your qualifying condition and your medical history.
